What all of these have in common
Each evaluation is independent. I assess your condition and report what I find. If the evaluation supports your request, you receive documentation that holds up. If it does not, I explain why and we talk about what else might help. The goal is documentation that is useful to you and credible to whoever reads it.
FMLA certification
$350 to $600
If a mental-health condition qualifies as a serious health condition under the Family and Medical Leave Act, you may be entitled to up to 12 weeks of job-protected leave. This evaluation assesses whether your condition meets that threshold and completes the required certification forms.
- Intermittent FMLA: $350 to $450 · 1.5-2.5 hours · forms within 72 hours
- Continuous FMLA: $450 to $600 · 2-3 hours · forms within 72 hours
Includes validated symptom-severity and functional-impairment measures. Completed forms go to you, not your employer. You decide who sees them.
ADA workplace accommodations
$450 to $750
If a mental-health condition limits your ability to do your job, you may be entitled to reasonable accommodations under the ADA. This evaluation documents your functional limitations and recommends specific changes: flexible scheduling, remote work, modified duties, reduced caseload, or whatever fits your situation.
The letter goes to your employer's HR department on letterhead. Your diagnosis can be withheld if you prefer; only functional limitations and recommended accommodations are required for the ADA interactive process.
Fee: $450 to $750 depending on complexity · 2-4 hours total · letter within 3-5 business days. Includes validated symptom-severity and functional-impairment measures.
SSI/SSDI support evaluation
$750 to $1,500
A detailed evaluation for Social Security disability claims, typically referred by your disability attorney. The report covers your diagnosis, symptom severity, what you can and cannot do functionally, and whether you can sustain competitive work.
Includes standardized symptom measures and a structured functional assessment addressing the domains used in SSA disability determinations. This is an independent clinical evaluation, not an SSA consultative exam. It produces a report your attorney submits as evidence.
Fee: $750 to $1,500 · 5-8 hours total · report within 5-10 business days.
For attorneys
Referrals from disability and employment attorneys are welcome. Reports include diagnostic rationale, functional assessment, and supporting documentation. Detailed evaluation methodology, instrument list, and CV available on request. Attorneys can refer via the request form or by direct email at owner@drsashasioni.com.
Private disability insurance (STD/LTD)
$500 to $1,000
If your employer-sponsored disability plan requires clinical documentation, this evaluation provides a diagnostic assessment, functional-impairment summary, and completed insurer forms. Periodic updates are available at $200-$400 each.
Fee: $500 to $1,000 · 3-6 hours total · documentation within 5-7 business days. Some insurers specifically require a psychiatrist or licensed clinical psychologist. Check your plan before scheduling.
Evaluator background
Dr. Sasha R. Sioni, DBH, MPH, LMHC-QS. Doctor of Behavioral Health (Arizona State University), Master of Public Health (Yale School of Public Health), Certificate in Professional Neuropsychological Assessment (UC Berkeley Extension). Florida Licensed Mental Health Counselor (MH24469) and Qualified Supervisor.
The UC Berkeley certificate is the official credential name. Services are provided as an LMHC, not as neuropsychological testing. The Florida LMHC license authorizes clinical assessment and diagnostic evaluation.

Common questions
Will my report hold up if challenged?
Reports include diagnostic rationale, validated symptom-severity and functional-impairment measures, and a structured functional assessment aligned to the evaluation type (FMLA serious health condition, ADA reasonable accommodation, SSA disability determination domains, or private STD/LTD plan criteria). Whether a report is accepted or given weight is ultimately determined by the employer, insurer, attorney, agency, or adjudicator. I do my work; how that work is received is outside my control.
Do you accept attorney referrals?
Yes. Disability and employment attorneys can refer clients via the request form or by direct email. Reports include diagnostic rationale, functional assessment, and supporting documentation. Detailed evaluation methodology, instrument list, and CV are available on request before referral.
What if I am not sure which evaluation I need?
Start with the request form and describe your situation in a few sentences (what you are trying to document, who is asking for it, any deadlines). A free 15-minute consultation is available to confirm fit and scope before the evaluation is scheduled.
Is this an SSA consultative exam?
No. The SSI/SSDI support evaluation is an independent clinical evaluation that produces a report your attorney can submit as evidence. It is not the same as the consultative exam SSA itself orders. Both can coexist in a case file.
How does payment work for ranges?
Each evaluation type has a fee range because total time varies with case complexity, document volume, and form length. The estimate is set at intake based on what you describe, with the upper bound reserved for cases that require additional time. HSA / FSA eligible; superbill provided on request.
