Good Faith Estimate

Under the No Surprises Act, uninsured and self-pay patients can request a Good Faith Estimate of expected charges before or at the time of scheduling. The estimate covers items and services that may be provided as part of your care. It is not a bill.

If you receive a bill that exceeds your Good Faith Estimate by $400 or more, you may be able to dispute it through the federal patient-provider dispute resolution process. Details are available at CMS: Dispute a medical bill.

To request a Good Faith Estimate, use the appointment request form or call (321) 295-1200.
