Please do not send health information or records by email. Referrals start with the patient's own secure inquiry. After intake, I request records with the patient's authorization.
A few brief criteria to help you assess whether a referral is appropriate.
Each evaluation uses a clinical interview, validated rating scales, and review of records the client authorizes. Reports and summaries are shared with you only with the client's signed authorization.
Adults and teens 12+. $1,295 for the diagnostic evaluation; $1,695 with accommodation documentation. The client receives a written summary for the prescriber within one week of the last session. I do not prescribe.
$450. With the patient's signed authorization, a clinical summary with a clearance recommendation goes to the surgical team within 3 to 5 business days of the evaluation session. Some programs and insurers require a psychiatrist or licensed clinical psychologist, so please confirm that your program accepts an evaluation from an LMHC.
FMLA certification, ADA accommodation letters, and private STD/LTD documentation. $350 to $1,000, depending on the type. Social Security (SSI/SSDI) evaluations are not offered. Attorneys can request my evaluation methods, instrument list, and CV before referring. The disability page explains who accepts documentation from a licensed mental health counselor.
Evaluations: virtual by default; in-person testing components arranged when an exam or instrument requires them. These are self-pay prices. Patients with Original Medicare or a plan I am in network with should ask before they schedule, because some parts may need to be billed to their plan.
Active scholarly interests: gaming disorder, internet gaming disorder, behavioral health integration in primary care, and population approaches to behavioral health. Peer-reviewed work includes internet gaming disorder (Computers in Human Behavior, 2017), a scoping-review protocol on digital behavioral addictions (PLOS ONE, 2026), and integrated care-management outcomes (Health Services Research, 2026).
