Before you write
A brief summary is enough. (Example: "problematic gaming," "anxiety," "relationship conflict.") If you're emailing directly, please don't include diagnoses, medication lists, or detailed health information — use the SimplePractice intake (above) for anything you'd prefer to send through a HIPAA-handled channel.
