A lot of people come in having spent years assuming the problem was something else — effort, discipline, anxiety. The evaluation answers whether ADHD is actually part of it, and gives your prescriber written documentation, typically within one week.

Why this evaluation costs $995

Some services offer ADHD assessments for $200–$400 based on a self-report questionnaire reviewed by a clinician you never meet. Those work for some people. This is different.

This evaluation includes a live clinical interview, validated rating scales, screening for conditions that mimic ADHD, and a written report your prescriber can act on. You talk to the clinician who writes your report. The whole process takes about two weeks, and the summary is in your hands within one week.

What the evaluation covers

The evaluation is designed to give your prescriber the clinical documentation they typically need to make their own treatment decisions.

  • In-depth clinical interview covering developmental history, current symptoms, and daily functioning
  • DSM-5-TR symptom mapping with childhood-onset and duration criteria
  • Functional impairment review in at least two settings (home, work, school, social)
  • Validated ADHD rating scales
  • Screening for mood, anxiety, sleep, trauma, and substance use
  • Review of relevant records and collateral information when available
  • Prescriber-oriented summary with safety and treatment considerations

Diagnosis is based on the full clinical picture, not a single questionnaire or test score. Optional performance measures like the CPT or executive functioning scales are used selectively when they would add useful information.

Certificate in Professional Neuropsychological Assessment (UC Berkeley Extension)

Official credential name. Services are provided as an LMHC, not as neuropsychological testing.

Delivery format

  • Telehealth — available statewide for clients located in Florida at the time of the evaluation
  • Mobile in-person — within 30 miles of Orlando, I come to you. The initial consultation is telehealth; the evaluation session itself is conducted in-person at your location.

For adolescents, a parent or guardian is involved in the evaluation process.

  • A clear diagnostic opinion (ADHD confirmed, ruled out, or deferred with rationale)
  • A written clinical summary formatted for your prescribing physician or psychiatrist, with diagnostic rationale, symptom severity, and treatment considerations

“Dr.” = academic doctorate (DBH). Services provided as a Florida-licensed mental health counselor (LMHC, MH24469). The training listed above qualifies him to conduct ADHD diagnostic evaluations using clinical interview, validated rating scales, and behavioral observation.

This is an evaluation, not ongoing therapy. Your written report may be shared with your prescriber or other providers you designate. Full informed consent, including limits of confidentiality, is provided before the evaluation begins.

Common questions

What rating scales do you use?
The evaluation uses validated, normed ADHD symptom measures designed for adult assessment, along with functional impairment inventories and brief mood/anxiety screening. Specific instruments are selected based on your age, history, and the referral question. A full methodology document is available on request.
Will my prescriber accept this?
The written summary includes diagnostic rationale, symptom severity, functional impact, and treatment considerations. Prescribers generally have what they need to make their own treatment decisions. Final prescribing decisions remain with the prescribing clinician.
How is this different from an online screener?
Online screeners use a single self-report questionnaire. This evaluation includes a clinical interview, multiple validated rating scales, records review when available, and screening for conditions that look like ADHD but are not. The result is a diagnostic opinion with written documentation, not a screening score.
Can I use my insurance for this evaluation?
No. ADHD evaluations are self-pay only and are not billed through insurance. HSA and FSA funds are eligible. A superbill is provided if you want to submit for potential out-of-network reimbursement, though reimbursement depends on your specific plan and is not guaranteed.
