What the evaluation includes
The evaluation is designed to be complete on its own — your prescriber receives everything they need without additional testing on their end.
- In-depth clinical interview covering your history, symptoms, and daily functioning
- Validated ADHD rating scales with a functional impairment review
- Review of relevant records and collateral information when available
Delivery format
- Telehealth — available statewide for clients located in Florida at the time of the evaluation
- Mobile in-person — I travel to you within 30 miles of Orlando for an additional fee; no need to visit an office
For adolescents, a parent or guardian is involved in the evaluation process.
What you receive
- A clear diagnostic opinion (ADHD confirmed, ruled out, or deferred with rationale)
- A written clinical summary formatted for your prescribing physician or psychiatrist — usable as-is, no follow-up required from you
Optional add-ons: CPT (+$150) and executive functioning rating scales (+$95–$125).
Evaluation fee
Includes the full evaluation (reported above), written clinical summary, and one follow-up to review results. Not covered by insurance.
Written summary delivered within 1 week.
